YOU MUST HAVE THE FOLLOWING ITEMS WITH YOU TO APPLY:

1. Proof of income for all members of the household.
2. Birth Certificates for all members of the household.
3. Social Security Card for all members of the household.
4. Photo I.D. for all members of the household over 16 years of age.
5. Proof of medical expenses (prescription drugs, supplemental insurance).
6. Proof of unpaid medical bills (physician charges, hearing aid, hospital, eyeglasses, etc.).
7. Checking and/or savings account bank statement (savings must show amount in account and current rate of interest).
8. Proof of Social Security payment (annual award letter), Pensions, etc.
9. Proof of all assets.
